This Insurance policy will be a rider/addition to your Disability Insurance.

  • This Mandatory Employer Insurance Policy is set-up to be FULLY FUNDED BY EMPLOYEES.
  • NYS has determined that starting JULY 1, 2017 deductions will be or can be taken from employees as a payroll deduction.
  • If EMPLOYER DOES NOT WANT employees to contribute to this new insurance policy, you must contact your payroll provider for the first payroll of JULY 2017.
  • This deduction will be 0.126% of wages up to a maximum of $1.65/week.
